New Patient Registration

If you fall within our catchment area (see map below) and would like to register at our surgery then you will need to complete a new patient questionnaire and have a “registration appointment” with the practice nurse. This will give us some background information, until your records arrive, on past medical problems and lifestyle. You may prefer to discuss parts of the questionnaire personally with the nurse. If you are on regular medication you will need an appointment with a doctor to ensure that you will be able to request further prescriptions.

Please note that when registering, you can only do this in person (this must be the person that wishes to register) as we cannot accept registrations over the telephone, by email or by post.

To download the necessary form for registration please click on the link below.  Once completed please bring it in to the surgery.  We accept registrations between 10.00am and 6.30pm 5 days per week.

Please ensure you print out both forms and bring them with you to the surgery when registering
